These are the four reference cards from Appendix A of the book, formatted for a single sheet of paper. Print them at 100 percent, one card per page, and put them where you will actually see them: the inside of an office door, the back of a clipboard, the wall above the schedule.
Letter is the default. A4 is there if you are outside North America. Both print at 100 percent, so do not let your printer scale to fit.
